Step #2

Waiver Screening/Approval: Once the individual has been screened and approved for one of the above-listed waivers/programs, our team will help with the next steps. They will also provide you with information to familiarize you prior to scheduling your first visit, so that you feel prepared for that first in-home visit with your service facilitator.

Step #3

Assigned Agency: Once our team at Heart of Hands Home Care Services has completed the intake and processing of your referral, your case will be assigned to the appropriate staff member. A representative from our agency will contact you by phone to introduce themselves, review your approved services, answer any questions, and coordinate the start of your care. We look forward to providing compassionate, dependable, and high-quality home care services while supporting you every step of the way.

Step #4

In Person Intake Home Visit: During this visit at the client’s home, the Registered Nurse works with the member and the Primary Caregiver to develop a personalized care plan that meets the member’s needs and ensures compliance. They also update all necessary documents, and obtain signatures on documentation required to submit to your assigned approving authority and fiscal agent. This visit must take place at the client’s home.

Step #5

At this point, Heart of Hands Home Care Services will submit the paperwork to the MCO for approval.
DO NOT ALLOW ATTENDANTS TO WORK PRIOR TO BEING APPROVED BY THE FISCAL AGENCY AND KNOWING HOW MANY HOURS WERE APPROVED BY THE APPROVING AUTHORITY (MCO/DMAS).
Within 30 days, you will be contacted to schedule your next home visit.
